These Terms of Use govern your access to and use of the Patient Familiarisation Program portal (the “portal”), operated by Inservio Home Pty Ltd (ABN 17 751 399 102) (“Inservio Home”, “we”, “us”) on behalf of Galderma Australia Pty Ltd (“Galderma”).

By activating your account, and each time you sign in, you agree to these terms.

Accounts are by invitation only. Healthcare professionals are invited to the program, and their details are checked before an account is issued. Program staff are invited by Inservio Home or Galderma.

If you are a healthcare professional, we use your AHPRA registration number and your email address to identify you in the portal.

Each account is for one person only. Sharing your sign-in details, or letting anyone else use your account, is not allowed. You must keep your password and your authenticator app secure. We may suspend or cancel your access if there is evidence of account sharing or misuse of sign-in details.

By activating your account, you agree to receive service emails from Inservio Home about your account and the administration of the program. These emails are limited to running the program.

When you use the portal, you must:

• use it only in line with these terms, the program terms and conditions that apply to you, and any instructions we give you;
• not let anyone else sign in with your account;
• not try to reach any part of the portal you are not authorised to use, or interfere with its operation or security;
• tell us as soon as you become aware of any breach of these terms, or any use of your account you did not authorise; and
• take responsibility for all activity on your account.

Any fraudulent or otherwise illegal activity may be grounds for suspending or cancelling your access.

The portal is not intended or designed to record or report adverse events. Report adverse events as set out in the website footer.

If you are a prescriber, you must also:

• be solely responsible for the accuracy, completeness and lawfulness of the information you enter in the portal, including your practice, clinic and patient details;
• keep your details up to date; and
• enter a patient’s information only with the consent the program requires, recorded as the portal asks.

We aim to respond to questions and complaints within two business days.

We may change, add to or remove parts of these terms at any time by publishing a revised version on this website or by otherwise telling you. By continuing to use the portal after a change takes effect, you agree to the changed terms. Please review these terms before each use of the portal.

If these terms, or any change to them, are not acceptable to you, you may ask us to close your account and stop using the portal.
